Perfect Family Project Participant Authorisation Form

Dear Participant:

Thank you for participating in the Perfect Family project initiated by artist Qizhi Che. This project aims to explore the emotional connection between young people and their families through material art and interviews. The artworks you create in the workshop and the audio or text materials you provide may be used for the following public art purposes:

1. Art book publication (including physical and digital books);

2. Art exhibitions, workshop displays, and audio playback;

3. Project research and dissemination materials (such as papers, websites, and lectures);

During the workshop, you may need to:

1. There will be mental health professionals present; please do not feel uncomfortable;

2. There is a possibility of emotional breakdowns; we will immediately pause the session;

3. Share your story honestly and express your emotions;

4. We will make audio and video recordings.

We solemnly commit to the following:

1. All content used will respect your personal privacy. If you choose to remain anonymous, we will not disclose your real name; 

2. All materials will be used solely for non-commercial artistic and research purposes; 

3. If future publications involve royalties or profit-sharing, we will negotiate and confirm with you;

Perfect Family Participatory Sculpture and Family Memory Workshop Guidelines
Approximate duration: 2 hours | Suitable for 1-3 participants

Part 1: Welcome and Introduction (10 minutes)

Explain the theme to participants and have them sign the agreement form.

Stage 2: Guided interview (15 minutes)

Reference questions:
‘Which space in your home had the greatest impact on you when you were a child?’

‘Is there anything about your family that has stayed with you?’

‘How would you describe the emotional atmosphere in your home?’

‘If you could rebuild your ideal family, what would it look like?’

Stage 3: Drawing Memories (20 minutes)
Invite participants to start drawing pictures of their families, informing them that these will be the sketches for their sculptures. Tell participants that they can draw spaces, people, objects, emotions, or anything else they like.

Part 4: Sculpture Creation (40 minutes)
Place the materials on the table in the centre and let the participants create their sculptures. During this time, invite them to continue talking about their families.

Stage 5: Presentation and Narration (15 minutes)

Set up a ‘keyword notebook’ (for exhibitions and books).
After each workshop, write down the participants’
keyword tags (select 3-5)
a representative emotional quote
a description of their work
my observations and inspiration (such as story convergence points, character archetypes)
